SCHOOL GUIDANCE FORMS

Form 1- Cumulative Records
(This template is given to pupils/parents at the start of each school year.  It has cumulative information about the pupil that can be useful for teachers, advisers and guidance counselors)

Form 4-Letter to Parents 
(This template is used to call the attention of the parents especially when they are needed to come to school)

Form 5-Referral Slip
(This template is used by teachers/advisers when referring  pupil/s to the guidance office)

Form 8- Consent Dropping Form
(When a student dropped out of school, it is important to fill this template for record keeping)
